Job summary

Hospice Services of Massachusetts is seeking a Per diem RN Case Manager to plan, coordinate, and direct hospice care for patients and families. This on-the-road role requires a valid MA RN license and a reliable vehicle.

You will supervise aides, assess patients, develop care plans, and collaborate with the interdisciplinary team to meet patient and family needs. Competitive hourly pay up to $50 with weekly pay and mileage reimbursement.

Qualifications

• Graduate of an accredited school of nursing.
• Active Massachusetts RN license.
• 1–2 years acute care experience; 1–2 years home care preferred.
• CPR certification.

Per diem Hospice RN Case Managers with Hospice Services of Massachusetts

We are seeking an experienced and compassionate RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families.

Up to $50 an hour, depending on experience, with weekly pay.

This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel to portions of Barnstable and Plymouth Counties.

Requirements

• Must possess an active and unencumbered RN license in Massachusetts
• 1-2 years of recent acute care experience in an institutional setting, and 1-2 years of recent experience in home care preferred
• CPR certified
