Residential Home Health and Residential Hospice is seeking a Hospice RN to provide end-of-life care in patients' homes in York Springs, PA. This full-time role includes a $10,000 first-year bonus and a 40-hour work week.

As a nationwide leader in home health and hospice, you will collaborate with an interdisciplinary team, conduct comprehensive assessments, manage symptoms, educate families, and document in EMR. Travel within the assigned territory is required.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Ability to drive to patient locations (~20% of day).

Responsibilities

• Deliver compassionate, patient-centered hospice care with an interdisciplinary team.
• Perform comprehensive patient assessments and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey.
• Document thoroughly in EMR and ensure compliance.
• Work independently, but stay connected with the team.
• Travel to patient homes within the assigned territory.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
