Residential Home Health and Residential Hospice is seeking a Hospice Registered Nurse (RN) to provide end-of-life care in patients' homes in the Brownstown area.

The ideal candidate holds an active RN license in the state, has at least two years of RN experience (hospice preferred), and can drive to patient locations as needed. This full-time role offers comprehensive benefits and a $10,000 first-year bonus.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team

• Educate and support patients and families through the end-of-life journey with clear information and empathy
• Document thoroughly in EMR and maintain compliance
• Work independently while traveling to patient homes within the assigned territory

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
