Job summary

AHN Healthcare@Home in Erie, PA seeks an experienced Hospice Registered Nurse to provide end-of-life care in patients' homes. You will work with an interdisciplinary team to ensure comfort, dignity, and quality of life, traveling to patient locations as needed.

Active RN license in state, at least two years of RN experience (hospice preferred). A $10,000 first-year bonus is available to milestone-achieving clinicians.

Qualifications

• Active Registered Nurse license in the state you work
• Two years of RN experience; hospice experience preferred
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed and cared for
• Think critically, solve problems in real time, ensure compliant EMR documentation
• Work independently, but collaborate with the team and travel to patient homes within territory

RN Qualifications
