Job summary

Allina Health is seeking a Hospice Resource RN for the Allina Hospice program in Minneapolis. This 1.0 FTE, 8-hour day shift role requires travel across the hospice territory to coordinate symptom management and care planning, supporting families and interdisciplinary teams.

The position is union-represented with occasional on-call. Requires ADN/ASN or vocational degree, 2+ years RN experience, and MN/WI licensure as applicable; preferred CHPN and CMGT-BC certifications.

Qualifications

• Associate's or Vocational degree required.
• 2+ years RN experience required.
• Current BLS certification (American Heart Association or Red Cross) or in-house BLS within 30 days of hire.
• MN or WI RN license upon hire as applicable.
• CHPN certification preferred.
• Valid driver's license required within 60 days of employment.

Responsibilities

• Perform comprehensive assessment of patient and family needs and establish plan of care.
• Coordinate and delegate to ensure safe, effective care.
• Provide education to staff to maintain high-quality hospice care.
• Act as charge nurse or preceptor when assigned.
• Travel within hospice territory to meet patient and program needs.
