Job summary

Agrace is seeking an RN Alt Shift to provide patient- and family-centered care across homecare teams in Walworth County and surrounding areas. This 0.7 FTE role (28 hours/week) is a PM shift from 5:00pm to 1:30am, with on-call rotations, weekends every other weekend, and three holiday rotations.

A $10,000 sign-on bonus applies to 0.7 FTE, with prorated amounts for lower hours. Ideal candidates hold a current Wisconsin RN license, have 1 year of nursing experience (preferred), and possess a valid

Qualifications

• Current, unrestricted Registered Nursing License in state of employment
• One-year nursing experience preferred
• Valid driver’s license, auto insurance, and access to a reliable vehicle
• Must be at least 18 years of age
• Basic Microsoft Office skills
• Intermediate electronic health records skills
• Ability to use proper body mechanics, safe patient handling, and standard precautions

Responsibilities

• Perform patient and family assessments, collecting data and documenting findings
• Provide patient visits, interventions, pain management, and education
• Develop and revise the plan of care with the Interdisciplinary Team and family
• Oversee and delegate tasks to care team members and participate in meetings
