Job summary

Superior Hospice, part of the Home Health Care, Inc. family, is seeking a Hospice Visit Nurse (RN) to provide in-home hospice care in Saint Paul, MN. This is a part-time role with the opportunity to transition to full-time as patient volume grows.

You'll perform routine visits, complete assessments, manage symptoms, educate families, and collaborate with an interdisciplinary team to deliver compassionate, patient-centered care in patients’ homes and care communities.

Qualifications

• Active Minnesota RN license required (from qualifications section).
• Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, med-surg, or another clinical setting with end-of-life care exposure.
• Excellent assessment, communication, and time-management skills.
• Ability to work independently in a field-based role.
• Reliable transportation and a valid driver's license.

Responsibilities

• Provide routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing settings.
• Complete nursing assessments, symptom management, medication education, and comfort-focused interventions.
• Support patients and families through education, advocacy, and compassionate communication.
• Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate, timely EMR documentation.
