Job summary

Superior Hospice is seeking a Weekend Hospice Visit Nurse to deliver expert clinical care in patients' homes, assisted living, memory care, and skilled nursing settings. You will assess and manage pain, respond to changes in condition, and provide compassionate education to families for continuity of care throughout the weekend.

Choose every weekend or every other weekend based on availability, with competitive hourly pay and mileage reimbursement.

Qualifications

• Active Minnesota RN license required.
• Hospice, oncology, palliative care, ICU or end-of-life care experience preferred.
• Ability to work independently in the field.
• Reliable transportation and a valid driver's license.

Responsibilities

• Complete routine hospice visits within assigned territory.
• Assess patients, manage pain and symptoms, and respond to changes in condition.
• Provide education and support to patients, caregivers, and families with compassionate communication.
• Coordinate care with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate, timely EMR documentation.
