Job summary

Superior Hospice, part of Home Health Care, Inc., is seeking an RN to provide compassionate hospice care in patients' homes across the Twin Cities Metro. You will perform assessments, symptom management, and education for patients and families, delivering high-quality care in home settings.

You will work independently with support from an experienced interdisciplinary team, ensuring thoughtful, skilled nursing care focused on one patient and family at a time.

Qualifications

• Active Minnesota RN license.
• Hospice experience welcome; ICU/ER/oncology/med-surg/home health/palliative care candidates encouraged.
• Strong clinical assessment and communication skills.
• Confidence working independently in the field.

Responsibilities

• Provide evening hospice visits to patients in the Twin Cities Metro.
• Perform assessments, symptom management, medication administration, and comfort-focused nursing care.
• Educate and support patients, families, and caregivers.
• Deliver care in homes, assisted living, memory care, and skilled nursing facilities.
• Coordinate closely with physicians and the interdisciplinary hospice team.
• Complete timely and accurate EMR documentation.
