Job summary

Superior Hospice, part of the Home Health Care, Inc. family, seeks an experienced RN to provide scheduled and PRN hospice visits in patients' homes and care settings in Little Canada, MN.

You will assess needs, manage symptoms, educate families, and document care in the EMR system. Active Minnesota RN license required; hospice experience preferred (home health, oncology, ICU or end-of-life care considered).

Qualifications

• Active Minnesota RN license.
• Hospice experience preferred; home health, oncology, ICU or other end-of-life care settings considered.

Responsibilities

• Provide scheduled and PRN hospice visits in patients' homes, assisted living, memory care, and SNFs.
• Assess patient needs; manage symptoms, provide education on medications, wound care, and comfort-focused nursing interventions.
• Educate and support patients and families throughout the hospice journey.
• Collaborate with the interdisciplinary team to ensure individualized, compassionate care.
• Complete EMR documentation in a timely and accurate manner.

What You'll Bring

• Active Minnesota Registered Nurse (RN) license.
  
• Hospice experience preferred, with consideration given to nurses from home health, oncology, ICU, palliative care, or other end-of-life care settings .
  
• Outstanding communication and relationship-building skills.
  
• Ability to work independently while managing a field-based schedule.
  
• Reliable transportation and a valid driver's license.
