Job summary

Superior Hospice is seeking a Hospice Visit Nurse (RN) in Blaine, MN to deliver compassionate, patient-centered care in home settings. This role starts part-time with a clear pathway to full-time growth as patient volume expands.

You will travel to patients' homes or local care facilities, perform assessments, and provide symptom and pain management with a focus on comfort and quality of life.

Qualifications

• Active Minnesota RN license.
• Hospice experience preferred or strong home health/end-of-life care background.
• Ability to work independently with a flexible field schedule.

Responsibilities

• Travel to patients' homes and facilities to provide routine and PRN hospice visits.
• Perform comprehensive nursing assessments and pain/symptom management.
• Deliver wound care, medication management, and comfort-focused nursing interventions.
• Educate families and caregivers throughout the hospice journey.
• Collaborate with physicians, social workers, chaplains, and the interdisciplinary team.
• Complete accurate EMR documentation while ensuring Medicare hospice compliance.

What You'll Do

• Travel to patients' homes, assisted living communities, memory care, and skilled nursing facilities to provide routine and PRN hospice visits.
• Complete comprehensive nursing assessments and provide expert pain and symptom management .
• Deliver wound care, medication management, and comfort-focused nursing interventions.
• Educate and support patients, caregivers, and families throughout the hospice journey.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and the interdisciplinary team .
• Complete accurate, timely EMR documentation while maintaining Medicare hospice compliance.

What You'll Bring

• Active Minnesota Registered Nurse (RN) license.
• Hospice experience preferred, or a strong background in home health, oncology, ICU, palliative care, med-surg, or another clinical setting with end-of-life care experience.
• Strong clinical assessment , communication, and critical thinking skills.
• Ability to work independently while managing a flexible field schedule.
• Reliable transportation and a valid driver's license.
