Job summary

PruittHealth Hospice is seeking a Registered Nurse Case Manager (RN) for Hospice Services in Georgia–LaFayette. This full-time, first-shift role supports patients at home or in care facilities, coordinating care and ensuring adherence to the individualized care plan.

You will evaluate and treat, document promptly, follow policies, and maintain continuity of patient care in a hospice setting. The position offers competitive pay, merit increases, PTO, and comprehensive health plans.

Qualifications

• Current, active, and unrestricted RN licensure in the state of practice.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Complete all documentation and records in a timely and accurate manner.

• Maintain continuity of patient care in a hospice setting.
