Job summary

PruittHealth is seeking a Hospice Registered Nurse Case Manager (RN) to join our hospice services team in North Carolina. The role focuses on delivering compassionate, patient-centered care in the patients' homes or care facilities, ensuring adherence to care plans and accurate documentation.

The ideal candidate will hold a current, active RN license in the state of practice and demonstrate commitment to high-quality hospice care, with opportunities for professional growth and strong team

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
