Job summary

Agape Hospice Care is seeking a compassionate Registered Nurse to administer skilled nursing care in patients' homes within Clarke & surrounding counties. The role coordinates care with an interdisciplinary team, patients, families, and referring agencies to ensure timely, high-quality hospice services.

The ideal candidate will have an RN license, CPR certification, strong communication, and organizational skills, plus 2 years of clinical nursing experience, preferably in hospice or home health.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; previous hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office suite experience preferred.
• Strong interpersonal and communication skills; ability to support patient and family.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency; manage care coordination.
• Perform comprehensive assessments of terminal disease impact on physical, functional, psychosocial, and environmental needs.
• Implement individualized Plan of Care and revise as necessary.
• Educate patient/family about disease process, self-care, end-of-life care, nutrition, and dietary needs; train other staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate patient/family services and prioritize needs with the IDT.
