Job summary

Agape Hospice Care in Georgia is seeking a Registered Nurse to administer skilled nursing care to patients at home and coordinate with the interdisciplinary team.

You will assess needs, implement the care plan, educate families, and provide on-call support to ensure comfort and quality end-of-life care.

A current RN license, CPR certification, and at least two years of clinical experience are required; hospice or home health experience is preferred; MS Office skills are a plus.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal and communication skills with patients and families.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Educate patient and family about disease processes, self-care, end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes reflecting goals and outcomes.
• Coordinate all patient/family services with the IDT and prioritize needs.
• Refer to other services as needed and use case management approaches.
• Inform physicians of changes in patient needs and outcomes; monitor responses.
• Determine scope and frequency of services based on acuity and family needs.
• Provide on-call responsibilities and on-call service as assigned.
• Other duties as assigned.
