Job summary

Agape Hospice Care in Georgia seeks a dedicated Registered Nurse to administer skilled nursing care at patients' homes, coordinate with the interdisciplinary team, and ensure compassionate end-of-life care. This role requires a degree in nursing, an active RN license, CPR, and strong communication skills.

Two years of RN experience in clinical care, hospice or home health preferred, with proficiency in computers and MS Office. On-call duties apply as scheduled.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer use; MS Office preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs and ADLs.
• Coordinate care with the interdisciplinary team and family.
• Educate patient/family about disease, end-of-life care, and self-care.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes and document care.
• Coordinate services with IDT and refer to other services as needed.
• Provide on-call service to patients/families as assigned.
