Job summary

Agape Hospice Care in Clarke County, GA is seeking an experienced registered nurse for skilled nursing care in patients’ homes. You will coordinate with the interdisciplinary team, patients and families to plan and deliver end-of-life care with compassion and clinical excellence.

The role requires an active RN license, CPR certification, and strong communication, organization, and computer skills. Experience in hospice or home health is preferred, along with a valid driver’s license.

Qualifications

• RN license in good standing and current state license.
• Two years’ RN experience in a clinical care setting; hospice/home health experience preferred.
• Current CPR certification.
• Proficient computer skills; MS Office experience preferred.
• Valid driver’s license.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs.
• Coordinate care with the interdisciplinary team and patient/family and referring agency.
• Educate patients and families about disease processes, self-care, end-of-life care, nutrition and dietary needs; provide training to other staff.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate patient/family services with the IDT and manage referrals.
• Inform physicians of changes in patient needs and outcomes of intervention.
• Determine scope and frequency of services based on acuity and needs.
• Perform on-call responsibilities as assigned.
