Job summary

Agape Hospice Care in Georgia is seeking a compassionate Registered Nurse to provide skilled nursing care in clients' homes, coordinate care with the interdisciplinary team, families, and referring agencies, and manage overall care coordination.

You will perform assessments, implement the plan of care, educate patients and families, train staff as needed, and participate in on-call coverage to ensure continuous, patient-centered hospice services.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and current driver’s license.
• Two years’ RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; MS Office experience preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Administer skilled nursing care for clients in their homes across ages.
• Coordinate care with IDT, patient, family, and referring agency.
• Implement and revise the Plan of Care as needed.
• Educate patient/family on disease, care needs, and end-of-life care.
• Provide training to staff as needed and assist with on-call duties.
• Prepare clinical and progress notes documenting goals and outcomes.
• Assess caregiver ability to meet patient needs and arrange support services.
