Job summary

HopeWest in Grand Junction, CO is seeking a Licensed Practical Nurse (LPN) to join the PACE team. This full-time role offers flexible scheduling options (4x10s or 3x12s with rotating weekends) and direct home-based nursing care to participants.

As an LPN, you will deliver hands-on care per care plans, monitor health status, administer medications, document accurately, and collaborate with RNs, providers, and other professionals to support aging adults living at home and in the community.

Qualifications

• Current and valid Colorado LPN license
• Graduate of an accredited practical nursing program
• Current Colorado driver’s license and reliable transportation
• Experience with older adults or complex medical needs preferred
• Strong clinical judgment and excellent communication and interpersonal skills
• Collaborative, flexible approach to patient care, compassionate and person-centered

Responsibilities

• Provide direct nursing care to PACE participants in their homes according to care plans
• Monitor participant health status and report changes to the clinical team
• Administer medications and treatments within LPN scope
• Collaborate with RNs, providers, CNAs, therapists and social workers
• Support participant education and promote independence, safety and well-being
• Maintain accurate and timely clinical documentation
• Follow regulations, policies and standards of care

PACE provides comprehensive, coordinated healthcare and supportive services that allow older adults with complex medical needs to remain living independently in their homes and communities for as long as possible. As an LPN, you will play an important role in delivering hands-on care, supporting participants and families, and collaborating with an interdisciplinary team committed to providing person-centered care.

What You'll Do

As a PACE LPN, you will provide direct nursing care and clinical support to participants in their homes. Responsibilities may include:

Provide direct nursing care to PACE participants in accordance with individualized care plans and provider orders.

Monitor participant health status and promptly communicate changes in condition to the appropriate clinical team members.

Administer medications and treatments within the scope of LPN practice.

Collaborate closely with RNs, providers, CNAs, therapists, social workers, and other members of the interdisciplinary team.

Support participant education and encourage independence, safety, and overall well-being.

Help ensure a welcoming, compassionate, and participant-centered experience.

Maintain accurate and timely clinical documentation.

Follow all applicable regulations, policies, procedures, and standards of care.

What You Bring

Current and valid Licensed Practical Nurse (LPN) license in the State of Colorado, maintained throughout employment.

Graduate of an accredited practical nursing program.

Current and valid Colorado driver's license, adequate automobile insurance and unlimited access to reliable transportation.

Previous nursing experience preferred; experience working with older adults or individuals with complex medical needs is a plus.

Strong clinical judgment and the ability to recognize and communicate changes in participant condition.

Excellent communication and interpersonal skills.
