Job summary

Optimal Hospice is seeking a PRN Registered Nurse to provide compassionate, patient‑centered care in Denver, Colorado. You will conduct comprehensive assessments, develop and revise care plans, and administer medications per physician orders.

You’ll collaborate with the interdisciplinary team and serve as case manager for assigned patients, ensuring continuity of care in a supportive environment. Requirements include an active RN license, NLN‑accredited nursing education, CPR certification, and

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Current valid driver’s license.
• At least one year of Hospice experience (preferred).

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient’s status by writing and initiating plan of care; re-evaluates as needed.
• Develops a care plan with goals based on nursing diagnosis and includes patient and family.
• Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs.
• Teaches patient and family self-care techniques and coordinates with the interdisciplinary group.
• Acts as Case Manager when assigned and coordinates care for the caseload.
• Attends interdisciplinary group meetings.
