Chapters Health System in Florida is seeking an experienced Registered Nurse to join the Hospice House team. The RN will assess patient and family needs, coordinate the Plan of Care with the IDG, and provide palliative and supportive care in the hospice setting.

Requirements include an active RN license, hospice or inpatient nursing experience, ability to use an EMR/EHR, strong communication in English, and a flexible schedule. Travel off-site may be required.

Qualifications

Current RN license in the state providing care.
Minimum one year inpatient nursing or hospice experience preferred for external candidates; internal applicants may have different requirements.
Experience with EMR/EHR systems preferred.
Ability to communicate effectively in English (verbal and written).
Must have BLS for healthcare professionals from AHA or Red Cross.

Responsabilités

Provides and manages direct care to patients and families as part of the IDG.
Coordinates the Plan of Care reflecting evolving patient needs.
Educates patient and family about disease process, palliative care, safety practices.
Reports changes in patient condition to IDG or other health professionals.
Participates in evaluating hospice referrals/admissions for level of care.
