Job summary

Affinis Hospice is seeking a dedicated Registered Nurse to provide professional nursing care for Hospice Services patients in Georgia. You will assess patient needs, coordinate with physicians, and maintain detailed records to support comprehensive care plans.

Qualifications

• Graduate from an accredited School of Nursing.
• Current Georgia RN license in good standing.
• Minimum six months acute care RN experience; home care/hospice preferred.
• CPR certification required.

Responsibilities

• Provide professional nursing care to Hospice Services patients.
• Identify patient/family needs and plan care per physician orders.
• Initiate coordination with physicians and Hospice staff.
• Maintain up-to-date patient records and documentation.
• Collaborate with IDG to develop care plans within 2 days of care start.
• Support patients/families through pain control and symptom management.
• Deliver guidance and counseling to patient/families during the dying process.
