Job summary

The role requires Georgia RN license, at least six months in acute care or relevant hospice experience, and ability to work with a team.

Qualifications

• RN license in Georgia (State of GA) in good standing.
• Graduate from an accredited School of Nursing.

Responsibilities

• Provide professional nursing care to Hospice Services patients.
• Identify patient/family needs and provide supportive care per physician orders and policies.
• Document observations, interventions, and evaluations in patient records.
• Coordinate with the Interdisciplinary Group to develop a comprehensive plan of care.
• Provide on-call coverage and assist with bereavement support as needed.
