Job summary

Affinis Hospice is seeking a dedicated registered nurse to provide professional nursing care to hospice patients across our care settings. You will assess, plan, implement, and evaluate nursing care with attention to pain and symptom management, while coordinating with the interdisciplinary team to meet patient and family goals.

The role requires GA RN licensure, good standing, CPR, and a minimum of six months acute care experience; hospice or home care experience is preferred.

Qualifications

• Graduate from an accredited School of Nursing.
• Current Georgia RN license in good standing.
• Six months of acute care RN experience preferred; hospice/home care preferred.
• Current CPR certification.

Responsibilities

• Provide professional nursing care to Hospice Services patients.
• Identify patient/family needs and provide supportive care per plan of care.
• Collaborate with Interdisciplinary Group to develop a comprehensive plan of care.
• Maintain up-to-date patient records with clear documentation of care.
• Participate in IDG conferences and on-call coverage as needed.
• Support patients and families through the dying process and bereavement.
