Job summary

The Care Team Hospice in Carmel, IN is seeking a compassionate Hospice RN Case Manager to coordinate patient care, manage plans of care, and work with physicians, caregivers, and the interdisciplinary team to ensure comfort and dignity.

This role features a Monday–Friday schedule (8:00 AM–4:30 PM) with on-call opportunities for additional compensation in a supportive culture offering growth and comprehensive benefits.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and physician orders.
• Complete scheduled visits, admissions, and interventions with accurate documentation in HCHB.
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support outcomes.
• Educate patients and families on symptom management, disease progression, and end-of-life care.
