Job summary

Elara Caring in Ohio seeks a Registered Nurse Case Manager to support patients in Fulton County. You will coordinate home-based care, assess needs, and collaborate with physicians to ensure quality outcomes.

The role requires active RN license, 1+ year in clinical nursing, and ability to travel between patient homes. Hospice or home health experience is preferred, and a valid driver’s license is essential.

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.
