Elara Caring is seeking a Registered Nurse Case Manager to support hospice and home health patients in Missouri. You’ll assess patient needs, coordinate care, and communicate changes to the care team while delivering compassionate, patient-centered support in the home.
The role requires an active RN license, 1+ year clinical experience, ability to lift up to 50 pounds, and a valid driver's license to travel between patient homes. Hospice or home health experience is preferred.
