Job summary

Elara Caring invites a dedicated Hospice Registered Nurse to join our team as a Full-Time RN Case Manager. You will assess patients and families, coordinate care, and educate staff across nursing homes, hospitals and home settings.

We seek an RN with a current license, 1+ year clinical experience, and willingness to travel for patient visits. Hospice/home health experience is preferred. Benefits include comprehensive insurance and 401(k) with employer match.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (certificate/diploma/associate/baccalaureate).
• 1+ year of nursing experience in a clinical setting.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Valid driver's license and reliable transportation for home visits.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to physician and care team.
• Educate patients, families, caregivers, and facility staff on the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management and symptom control to staff in various care settings.
• Offer compassionate support at end of life and complete bereavement assessments for families and caregivers.

As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred
