Job summary

Elara Caring is seeking a dedicated Registered Nurse Case Manager to support hospice and home health patients across assigned locations. You will assess needs, monitor conditions, and educate families and staff to ensure high-quality, compassionate care in the home setting.

The role requires an active RN license, 1+ year clinical experience, and willingness to travel between patient homes. Prior hospice experience is preferred and benefits include comprehensive plans and 401(k) matching.

Qualifications

• Current state license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to lift up to 50 pounds and travel between patient homes.
• Experience in hospice or home health is preferred.

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.
