Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our hospice team and deliver High-Quality care in patients' homes. You will assess needs, monitor conditions, and coordinate with physicians and caregivers to implement the interdisciplinary Plan of Care.

Ideal candidates have an active RN license and at least 1 year of clinical nursing experience, with willingness to travel between patient homes; hospice/home health experience is preferred.

• Graduate of an accredited Nursing program with 1+ year of clinical nursing experience.
• Ability to lift/push/pull up to 50 lbs and assist with patient transfers.
• Must have reliable transportation or access to travel between patient homes.
• Hospice or home health experience preferred.

As a Registered Nurse Case Manager, you'll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.
