Elara Caring is seeking a compassionate Registered Nurse Case Manager to support hospice patients and families in a home health setting. You will assess needs across physical, psychological, social, spiritual, and environmental domains and coordinate care with physicians and the care team.

You will educate staff on hospice principles, pain and symptom management, and provide bereavement support as part of a dedicated interdisciplinary team. A valid RN license and driving support are required.

Qualificações

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and ability to travel between patient homes and assigned work locations.

• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.
