Zusammenfassung

Elara Caring is seeking a Registered Nurse Case Manager to lead hospice/home health care, assessing patients and families and coordinating interdisciplinary care across visits and settings.

You will monitor conditions, educate staff and families, provide pain and symptom management education, and support during bereavement. RN license, 1+ year experience, and travel are required.

Qualifikationen

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred
