Allegheny Health Network AHN Healthcare@Home is seeking a Hospice Registered Nurse Case Manager to provide end-of-life care in patients’ homes in the Coraopolis, PA area. The role offers a $10,000 first-year bonus and autonomy within an interdisciplinary team built on compassion and quality of life.

You will perform detailed patient assessments, manage symptoms, educate families, and document care in the EMR. Travel within the territory is required, with strong support for professional growth.

Qualifications

• Active RN license in the state you work
• Two years RN experience; hospice experience preferred
• Valid driver’s license and ability to drive ~20% of workday

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
