Job summary

AHN Healthcare@Home in McCandless Township seeks an experienced Registered Nurse (RN) to provide end-of-life hospice care in patients' homes. This role offers autonomy, meaningful patient relationships, and a $10,000 first-year success bonus.

You will perform patient assessments, manage symptoms, educate families, and document care in EMR. Travel to assignments within your territory is required; a valid RN license and driver's license are essential.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to drive to patient locations ~20% of time.

Responsibilities

• Deliver compassionate, patient-centered hospice care at patients’ homes.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey and ensure thorough EMR documentation.

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
