Job summary

Brighton Hospice is seeking an experienced Registered Nurse (RN) Case Manager to deliver compassionate hospice care in home and facility settings across Wisconsin. You’ll collaborate with an interdisciplinary team to develop individualized care plans, balance visits with documentation, and ensure comfort and dignity for patients and families.

Qualifications

• Current Registered Nurse license in good standing.

As a Registered Nurse (RN), you'll provide compassionate, individualized hospice care wherever patients call home—including private residences, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves. Depending on your strengths and interests, we do have various nursing opportunities available.

Registered Nurse - Case Manager

• Full Tim, Days - Monday – Friday

• Pay range: $78K - $83K/yrly DOE plus bonus opportunities

Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice

• A passion for providing exceptional, patient-centered hospice care

• Strong collaboration and communication skills with interdisciplinary care teams

• Ability to work independently while effectively managing your daily schedule

• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs

• Comfort learning and utilizing electronic medical record (EMR) systems

• Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area
