Job summary

Park Nicollet Hospice seeks a Referral and Scheduling Registered Nurse to coordinate referrals and scheduling across hospice and home care services. You will collaborate with interdisciplinary teams to ensure timely admissions and balanced caseloads, while supporting patients and families with compassionate care.

The role emphasizes strong communication, organization, and problem-solving abilities, with a schedule Monday–Friday and weekend coverage as needed. MN/WI RN licensure is required.

Qualifications

• Registered nurse with strong communication and coordination skills.
• Experience in hospice, palliative care, home care, or community care services is preferred.

Responsibilities

• Coordinate referrals and scheduling across Home and Community Services.
• Manage referrals through admission and ensure visits are scheduled efficiently.
• Collaborate with interdisciplinary teams and cover when needed.
• Support patients, families, and staff through regulatory and clinical processes.
