Qualifications

• Bachelor's degree in nursing with 2+ years of hospice, palliative care, home care, or other community care service experience.
• Associates degree in nursing with 4+ years of hospice, palliative care, home care, or other community care service experience.
• MN or WI Registered Nurse licensure; renewal fees paid for the second license.

Responsibilities

• Coordinate referrals through admission and scheduling across Home and Community Services.
• Collaborate with interdisciplinary teams to ensure appropriate visits and coverage.
• Serve as a resource for clinical, reimbursement, and regulatory matters.
• Focus on Hospice referral and scheduling.
