HealthPartners Hospice is seeking a registered nurse to assess and coordinate home- and community-based patient care, delivering skilled nursing and education to patients and families in line with community standards. You will improve or maintain health status, support terminal illness care, and prepare families for self-directed care.

The Hospice registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.
The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.
The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.
8-4:30pm days, or ability to cover night call 4:30pm-0800am to cover PTO replacement needs
