Job summary

Park Nicollet Health Services is seeking a Registered Nurse for our Hospice team in Saint Louis Park, MN. You will assess patient care needs and provide skilled nursing care and teaching to patients and families in home or community settings, coordinating resources to create a comprehensive plan of care.

You will support admissions, discharge, billing, referrals, bereavement, and delivery of care, working a daytime schedule with on-call coverage for PTO needs.

Qualifications

• Two years of RN experience required.
• Current MN Registered Nursing License.
• Knowledge of age-related growth and development principles.
• Experience operating basic office equipment and EMR systems.

Responsibilities

• Assess and evaluate patient care needs; provide skilled nursing care and education.
• Coordinate medical, social, and financial resources to develop a care plan.

Position Summary

The Hospice registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice. The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting. The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.

Work Schedule

8-4:30pm days, or ability to cover night call 4:30pm-0800am to cover PTO replacement needs

Required Qualifications

• Education, Experience or Equivalent Combination:
  • Work experience as a RN for at least two years
• Licensure/ Registration/ Certification:
  • Current MN Registered Nursing License
• Knowledge, Skills, and Abilities:
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
  • Proven proficiency in the operation of the following: Personal Computer, Telephone, Fax Machine, Photocopier, Printer, and electronic medical record.

Preferred Qualifications

• Experience in home care and or hospice desirable.
• Must be able to function independently with minimal supervision.
• High level of sensitivity, assessment skills, technical-skills and communications skills.

Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
