Job summary

Agape Care Group is seeking a Hospice Account Executive in South Carolina to grow patient referrals and nurture relationships with hospitals, physicians, and care teams. You will educate providers on hospice eligibility and coordinate transitions with clinical partners to ensure compassionate end-of-life care for patients and families.

Ideal candidates bring healthcare sales experience, knowledge of hospice or home health, and a passion for serving communities.

Qualifications

• Heart to serve patients and families; passion for providing the best possible care.
• Associate Degree or proven work experience in a health-related field required.
• 2 years of sales experience in a clinical care setting; hospice preferred.

Responsibilities

• Build and maintain relationships with hospitals, physicians, case managers, and referral partners.
• Generate new hospice referrals within assigned territories and existing accounts.
• Educate healthcare professionals about hospice services and patient eligibility.
• Coordinate with internal clinical teams to ensure smooth patient transitions into hospice care.
