Alc-Health-Care-Services is seeking a Hospice Referral/Intake Specialist to manage patient referrals, verify insurance, and coordinate transitions to hospice care. You will build relationships with referral sources and ensure regulatory compliance.

Job Title/Position: Hospice Referral/Intake Specialist
Reports To: Executive Director/Administrator
Responsible for performing all aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and conducting the insurance verification and authorization process.
1. Performs all daily patient referral and intake operations.
2. Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
3. Establishes and maintains positive working relationships with current and potential referral sources.
4. Ensures seamless transition of patients to hospice by coordinating care with multiple service providers.
5. Ensures maximum third-party reimbursement by conducting the insurance verification and authorization processes.
6. Builds and monitors community and customer perceptions of ALC Palliative and Hospice Care as a high-quality provider of services.
7. Gathers, collates, and reports referral statistics including key customer referral trends.
8. Maintains comprehensive working knowledge of ALC Home Health Care, Inc. contractual relationships and ensures that patients are admitted according to contract provisions.
9. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by ALC Home Health Care, Inc.

1. High School diploma or equivalent.
2. At least three (3) years’ experience in health care, preferably in hospice or palliative operations.
3. Demonstrated ability to supervise and direct professional and administrative personnel.
4. Ability to deal tactfully with customers and the community.
5. Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practice preferred.
6. Demonstrates good communication skills.
7. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
