Job summary

Amedisys, part of the Optum family, invites you to join our home health and hospice care team. The Hospice Liaison drives census growth by building relationships with physicians, hospitals, and community providers while educating partners on hospice services and ensuring smooth transitions.

Responsibilities include coordinating admissions, visiting sites to assess needs, maintaining CRM records, and achieving referral and payor-mix goals. Travel across the territory is a key part of the role.

Qualifications

• 1+ year of experience in a healthcare related business development/outreach role.
• Reliable transportation, current driver’s license, and liability insurance.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions with the team and manages required documentation for referrals.
• Conducts on-site visits to assess patient needs and identify barriers to care.
• Keeps accurate records of patient interactions and referrals in CRM.
• Achieves admissions, product, and payor-mix goals.
• Meets short and long-term target account goals.
• Documents the minimum expectation of sales calls daily in CRM.
• Maintains a complete, up-to-date record of targeted referral sources in territory in CRM.
• Attends community events to promote Amedisys.
• Responds to customer complaints promptly.
• Performs other duties as assigned.
• Note: This role will travel 50% or more within the assigned territory.
