Job summary

Amedisys is a leading home health and hospice provider seeking a hospice liaison to drive outside sales and develop key referral networks. The role focuses on educating healthcare professionals, patients, and families about home care services and coordinating admissions with the care team.

Qualifications

• At least one year of experience in a healthcare-related business development/outreach role.
• A bachelor’s degree is preferred but not required.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with referral sources to generate leads.
• Coordinates patient admissions with the team and completes referral processes.
• Conducts on-site visits to assess patient needs and barriers to care.
• Maintains records of patient interactions and referrals and achieves admission goals.
