Heartland Hospice in Fond du Lac, WI seeks a Hospice Care Consultant to educate referral sources, develop key relationships, and grow patient admissions. You will serve as a brand ambassador, presenting services with compassion and strategic insight to hospitals, home health agencies, SNFs, clinics, and physician offices.

The role requires a degree in business/marketing/communications, 2+ years healthcare sales, proven ability to meet targets, and proficiency with Office and CRM tools.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, skilled nursing facilities or home health professionals.
• Proficiency in Microsoft Office Suite and CRM platforms.
• Sensitivity to the needs of terminally ill patients and their families.

Responsibilities

• Educate, inform, and inspire confidence in our services to ensure timely referrals.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice options.
• Plan and execute strategic marketing initiatives to increase awareness and drive referral growth.
• Represent the organization at community events and educational in-services to promote our services.
