Job summary

Chapters Health is seeking a Per Diem Registered Nurse to assess patient and family needs, develop and implement the Plan of Care with the IDG, and provide palliative and supportive care across hospice settings.

Ideal candidates hold an RN license in the state, active BLS, at least 1 year in hospice or hospital settings, and experience with EMR/EHR systems. A valid driver's license and reliable transportation are required to meet visit schedules.

Qualifications

• Current license as RN in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year nursing experience in hospice or hospital preferred.
• Experience using an EMR/EHR system.
• Must have a valid driver's license and auto insurance per policy.

Responsibilities

• Performs comprehensive nursing care and assessments for patient/family in hospice setting.
• Coordinates Plan of Care with Interdisciplinary Group (IDG) and initiates appropriate nursing interventions.
• Delivers palliative and supportive care to patients and families.
• Completes documentation per policy and communicates with physicians and staff.
• Represents the company professionally at assigned facilities and with referrals.
