Job summary

Ohio’s Hospice is seeking a compassionate Hospice IPU PCS to deliver high-quality direct patient care in the Dayton area, with travel to patient homes and care facilities in the Zanesville region. This full-time role runs 7:00 am–7:30 pm with every-other weekend and holiday rotations.

Qualified candidates have a high school diploma or GED; a Certified Nurse Aide is preferred (CHPNA certification preferred).

Qualifications

• High school diploma or GED required.
• Certified Nurse Aide preferred; internal training offered.
• CHPNA certification preferred; Ohio Hospice supports certification.
• Proficient documentation in electronic medical records.
• Valid Ohio driver’s license with a safe driving record.
• Criminal background check and 10 panel drug screen.

Responsibilities

• Provide direct patient care and support families as part of an interdisciplinary team.
• Travel to patient homes and care facilities within the service region.
• Accurately document patient care and services in electronic medical records.
• Work full-time with 7am–7:30pm shifts and every-other weekend/holiday rotation.
