Qualifications

• High School Diploma or GED required or equivalent.
• Certified Nurse Aide preferred; internal training available.
• CHPNA certification preferred. Ohio’s Hospice supports growth.
• Proficient computer skills to document patient care in EMR.

Responsibilities

• Provide direct patient care to hospice patients in facilities and homes.
• Document patient care and services accurately in electronic medical records.
• Support families and work with an interdisciplinary team to deliver compassionate care.
