Job summary

Ohio’s Hospice is seeking a Hospice IPU PCS on a PRN basis for the Dayton area. The role supports patients and families with comfort, dignity, and peace, traveling to homes and facilities within the Zanesville service region.

Hours are 7am-7:30pm on a day-shift basis and may extend to Troy/Middletown IPU locations as needed. Qualifications include a High School Diploma or GED; CNA preferred with internal training; CHPNA certification preferred.

Qualifications

• Proficient computer skills to document patient care and services in EMR.
• Valid Ohio driver’s license with safe driving record and proof of insurance.

Responsibilities

• Provide compassionate care to patients in their homes and care facilities.
• Travel to patient homes and care facilities throughout the service region to support the interdisciplinary team.
