Job summary

Andwell Health Partners is seeking a Patient Service Navigator for Hospice to coordinate communication, scheduling, referrals, and documentation in a fast-paced in-person office setting.

You will be the primary contact for patients and families, manage referrals, and support clinical staff with timely information and accurate record-keeping. A high school diploma and scheduling experience are preferred.

Qualifications

• High school diploma required.
• Prior office or healthcare scheduling experience preferred.
• Electronic Medical Records experience a plus.
• Strong computer skills with Microsoft Word, Excel, Outlook, and PowerPoint.
• Excellent customer service and professional demeanor.

Responsibilities

• Serve as the first point of contact for patients, families, and providers.
• Manage scheduling of clinical staff visits in a busy office environment.
• Ensure appropriate assignment of referrals in consultation with supervisors.
• Develop and maintain documentation systems for timely coordination of tasks.
• Enter data into EHR and maintain staff timesheets.
