Job summary

Andwell Health Partners is seeking a Patient Service Navigator for Hospice to support patients, families, providers, and clinical staff in a fast-paced, in-person office setting. You will coordinate communications, referrals, scheduling, and documentation to ensure compassionate, efficient care delivery.

Requirements include a high school diploma, healthcare scheduling experience preferred, and strong Microsoft Office skills.

Qualifications

• High School diploma required.
• Prior office or healthcare scheduling experience strongly preferred.
• Electronic Medical Records experience a plus.
• Strong computer skills with Microsoft Word, Excel, Outlook, and PowerPoint.
• Excellent customer service and professional demeanor.

Responsibilities

• Serve as the first point of contact for patients, families and providers.
• Manage scheduling of clinical staff visits in a busy office environment.
• Ensure appropriate assignment of referrals in consultation with supervisors.
• Develop and maintain team documentation systems for accuracy and efficiency.
• Keep up-to-date knowledge of insurance forms and chart paperwork requirements.
• Prioritize team paperwork daily/weekly/monthly.
• Review paperwork for completeness of forms.
• Coordinate multi-discipline documentation timely.
• Perform data entry into the EHR and staff timesheets.
• Maintain daily/holiday/weekend schedules.
